The 11 questions in the inspection

Every question is answered Yes, No or N/A. Each No is flagged and asks for a photo and a comment, so the finding arrives with its evidence.

Slings

  1. 1The identification tag is readable and shows the working load limit
  2. 2There are no cuts, broken wires, kinks, knots or heat damage

Hooks and shackles

  1. 3Hooks have working safety latches
  2. 4Hooks are not bent, opened out or worn
  3. 5Shackle pins are correct and fully seated
  4. 6Working load limits are marked and readable

Chains

  1. 7There are no stretched, twisted or nicked links
  2. 8End fittings are undamaged

Records and storage

  1. 9The periodic thorough examination is in date
  2. 10Damaged gear is removed from use and tagged
  3. 11Gear is stored dry and off the floor

When to use it

  • Once a month.
  • The person responsible for the area or equipment answers each question on the spot.
  • Each record starts with: Equipment ID.

How it is scored

The template is built with points per answer and a pass mark of 90. In this library every audit is set to pass at 80 and every inspection at 90. Each submitted inspection shows its score and pass or fail, and every No is flagged with its photo and comment.
