The full question list

The 9 questions in the inspection

Every question is answered Yes, No or N/A. Each No is flagged and asks for a photo and a comment, so the finding arrives with its evidence.

Location and marking

  1. 1Every e-stop can be reached easily and nothing blocks it
  2. 2E-stops are clearly marked (red button on a yellow background)

Function test

  1. 3Pressing each e-stop stops the machine straight away
  2. 4Pull cords and trip wires stop the machine when pulled
  3. 5Guard interlocks stop the machine when opened
  4. 6Resetting an e-stop does not restart the machine by itself
  5. 7The stopping time is acceptable for this machine

Follow-up

  1. 8Any e-stop that failed was tagged out and reported
  2. 9The test result is recorded

When to use it

  • Once a week.
  • The person responsible for the area or equipment answers each question on the spot.
  • Each record starts with: Machine ID, Number of e-stops tested.

How it is scored

The template is built with points per answer and a pass mark of 90. In this library every audit is set to pass at 80 and every inspection at 90. Each submitted inspection shows its score and pass or fail, and every No is flagged with its photo and comment.

Change the points or the pass mark after you clone it. How rules and scoring work.

Use this template in LeanSuite
