The 10 questions in the inspection
Every question is answered Yes, No or N/A. Each No is flagged and asks for a photo and a comment, so the finding arrives with its evidence.
Device
- 1The device is clean, undamaged and aligned
- 2Mounting is secure and positioned as set up
- 3Indicator lights show normal status
Test
- 4A test piece interrupts the beam or sensor at each height or zone and the machine stops
- 5The machine does not restart until reset as designed
- 6Muting or bypass functions are used only as intended
- 7Safety distance still suits the stopping time
Records
- 8The test result is recorded
- 9Failed devices were tagged out and reported
- 10The latest full safety check is in date
When to use it
- Once a week.
- The person responsible for the area or equipment answers each question on the spot.
- Each record starts with: Machine ID, Device ID.
How it is scored
The template is built with points per answer and a pass mark of 90. In this library every audit is set to pass at 80 and every inspection at 90. Each submitted inspection shows its score and pass or fail, and every No is flagged with its photo and comment.
