Safety interlock test
Weekly test that guard interlocks stop the machine and cannot be easily bypassed.

The 11 questions in the inspection
Every question is answered Yes, No or N/A. Each No is flagged and asks for a photo and a comment, so the finding arrives with its evidence.
Physical check
- 1Every interlock switch is in place, secure and undamaged
- 2Guards close fully and line up with the switch
- 3Switches are not taped, jumped or defeated
- 4Cables and connectors are in good condition
Function test
- 5The machine will not start with the guard open
- 6Opening a guard while running stops the machine
- 7The machine does not restart on its own when the guard is closed
- 8Stopping time is acceptable
Follow-up
- 9Any failed interlock was tagged out and reported
- 10The machine was not used until the interlock was fixed
- 11The test is recorded and signed
When to use it
- Once a week.
- The person responsible for the area or equipment answers each question on the spot.
- Each record starts with: Machine ID, Number of interlocks tested.
How it is scored
The template is built with points per answer and a pass mark of 90. In this library every audit is set to pass at 80 and every inspection at 90. Each submitted inspection shows its score and pass or fail, and every No is flagged with its photo and comment.
