See it, say it, fix it is a simple way to teach hazard and near miss reporting: notice the hazard, report it, and fix it if it is safe to do so, or make the area safe and get help. This poster explains each step in plain words, with blanks for where your team reports and where the near miss cards are kept.

Hazards and near misses: every one reported is an injury that doesn't happen.
- See it Spot hazards and near misses. Spills, blocked exits, missing guards, damaged tools. A near miss is a warning: nobody was hurt this time.
- Say it Report it straight away. Stop anyone in danger first. Say what, where and when. Reporting is never blamed.
- Fix it Fix it only if it is safe and you are trained. If not, make the area safe, mark it and get help. Check that it stays fixed.

How to use it

Put it next to the near miss card box or the QR code people scan to report.

Fill in the blanks before you put it up, so nobody has to ask where to report.

Open a team huddle with one recent report and what was done about it.

Use it with the safety cross calendar to show the reporting habit growing.
Which size to print
- 24 x 36 in or A2: a main wall or team board, read from a few metres away.
- 18 x 24 in or A3: a station or notice board. The A3 file also fits 11 x 17 in paper.
- Letter or A4: a clipboard, binder or handout, on any office printer.
Every PDF is vector, so it stays sharp at full size. Nothing runs to the edge of the page, so no bleed is needed.
