Safety management
Safety management in manufacturing works best when it is proactive: finding and removing hazards before anyone is hurt, instead of investigating after an incident. Near-miss reporting is its most useful early signal, but only when each report leads to a removed hazard and people can see that it did; otherwise they stop reporting.
