QA matrix poster

A QA matrix (quality assurance matrix) puts every known defect, from customer complaints and internal checks, on one page against the process steps: where each defect is made and where it is caught. Each line is scored on frequency, severity, detection and cost and ranked, so the team works the defects that matter most instead of the loudest. This poster sets out the six steps to build one and keep it live.

The words on the poster

QA matrix: every defect on one page

Where each defect is made, where it is caught, and which to fix first.

  • List every defect Customer complaints and returns, and internal defects.
  • Map it to the process Which step makes it? Which step should catch it?
  • Score it How often, how severe, how well it is detected, what it costs.
  • Rank it Highest score first. Safety defects always at the top.
  • Work the top Root cause, error-proofing, better detection at the source.
  • Keep it live Re-score after each fix and each new complaint.
  • Made here, found there? A defect caught far from where it is made is a detection gap. Move the check to the source.

How to use it

Putting the QA matrix poster to work

  1. 1

Hang it by the quality board where the matrix is reviewed.

  2. 2

Build the first matrix in a workshop with operators, quality and engineering.

  3. 3

Look for defects found far downstream from where they are made: those are detection gaps.

  4. 4

Pair it with the free QA matrix template and defect detection matrix.

Which size to print

  • 24 x 36 in or A2: a main wall or team board, read from a few metres away.
  • 18 x 24 in or A3: a station or notice board. The A3 file also fits 11 x 17 in paper.
  • Letter or A4: a clipboard, binder or handout, on any office printer.

Every PDF is vector, so it stays sharp at full size. Nothing runs to the edge of the page, so no bleed is needed.

A poster reminds. A routine makes it stick.
