Defect escalation poster (red tag and stop the line)

When someone finds a defect, the first few minutes decide whether it stays at one part or reaches the customer. This poster sets out what to do: stop, red-tag and contain the suspect parts, escalate, record and restart only when it is safe to. An escalation ladder with blank times lets each site write in how long a problem can stay at each level before it goes up.

Stop it here. Don't let it reach the customer.

  • Stop Stop the station if you are allowed to. Call your team leader.
  • Contain Red-tag the part and quarantine it. Check the parts before it.
  • Escalate Use the ladder. Don't wait to be asked.
  • Record What, where, when and how many.
  • Restart Only when the cause is contained and the first part is good.

  • Not sure? Tag it and ask. Never pass on a part you are unsure about.

How to use it

    Agree the escalation times with production, quality and engineering, then write them in.

    Post it at every station and at the quarantine or red-tag area.

  3. 3

    Check at the huddle: did yesterday's problems go up the ladder on time?

  4. 4

    Pair it with the free escalation matrix, quality alert and NCR templates.

Which size to print

  • 24 x 36 in or A2: a main wall or team board, read from a few metres away.
  • 18 x 24 in or A3: a station or notice board. The A3 file also fits 11 x 17 in paper.
  • Letter or A4: a clipboard, binder or handout, on any office printer.
