Best near miss reporting software for manufacturing

Short answer: Near miss reporting software either sits inside an EHS suite or on the shop floor next to daily operations. The EHS suites here are Benchmark Gensuite, Cority, EHS Insight, Intelex and VelocityEHS: each takes near misses from a mobile app and runs them through investigation, CAPA, OSHA records and safety analytics, and Intelex and EHS Insight publish entry prices. Mitti (formerly SafetyCulture) is the low-friction option, with QR code reports that need no account and a published per-seat price. Voxel is different: its cameras detect forklift and pedestrian near misses that nobody reports. LeanSuite treats a near miss as a tag in its daily issue system: operators scan a QR code and report with a photo, with no app or login and anonymously if they choose, and a serious report becomes a safety CAPA in the same system as the plant's other issues and improvement projects.

How we chose these tools

Near miss reporting software lets anyone on the floor report an event that could have hurt someone but didn't, or an unsafe act or condition, in a minute or less. It sends the report to someone who can fix it, tracks the fix to closure and shows where reports cluster. Each product on this list meets all of these:

Eight near miss reporting tools: LeanSuite first, then alphabetical

  1. 1. LeanSuite

    Lean manufacturing software where a near miss is a tag type in Issue Management. Operators scan a station QR code and log a near miss, unsafe act or unsafe condition with a photo in their browser, with no app download and no login. The code fills in location, department and shift and shows open tags, so people add to one instead of duplicating it. Reports can be anonymous, Lean AI describes the photo and lists the hazards it sees, and rules route each report by type, area and priority to an owner with a due date, with push or email alerts to EHS; the reporter is told when it is fixed. A serious report becomes a CAPA project with approval steps, containment tasks, a 5-Why or fishbone the AI RCA Agent can draft and a time-stamped sign-off at each step. Heatmaps by zone, machine and department show where hazards cluster, and reporters can earn gamification points. One flat price per facility with unlimited users; a 90-day pilot costs $3,000 per facility.

    Best for: Plants that want near misses reported in seconds from the floor and worked through to a closed CAPA in the same system as their daily issues and improvement projects.

  2. 2. Benchmark Gensuite

    An EHS and risk platform from Benchmark Digital Partners with a Near Miss Reporting app, also called Concern Reporting. Employees report hazards, potential issues or near misses on mobile forms that work offline, and site and department leaders get automatic alerts. Critical concerns can be assigned for investigation and passed to Incident Management for causal analysis and corrective actions, and real-time analysis shows trends and leading indicators. Its Describe-It AI Advisor helps people write clearer descriptions, and AI flags potentially serious incidents. The same platform covers audits, training, CAPA and sustainability, with iOS and Android apps. Its pricing page lists Ready, Quick Start and Enterprise plans behind a Get a Quote button. It says it holds SOC 1, 2 and 3 reports and ISO 27001 certification, and offers single sign-on.

    Best for: Large multi-site manufacturers that want employee concern reporting inside a wide EHS, quality and sustainability suite.

  3. 3. Cority

    CorityOne, an enterprise EHS platform from a company that says it has built EHS software for 40 years. Its incident management captures incidents, near misses and observations as they happen. Workers report through myCority, which Cority describes as a secure mobile web app that works online or offline, and each submission starts workflows and notifications. Investigations follow guided methods, and its Cortex AI surfaces root cause patterns and drafts CAPA recommendations for a person to review. Reporting runs on Qlik Sense dashboards with trends and predictive insights. The suite also covers occupational health, environment, quality, eLearning and audits. Pricing is scoped to each organization on an initial three-year term and is not published. Cority lists ISO/IEC 27001, 27017 and 27018 certifications and FedRAMP-authorized infrastructure.

    Best for: Large or regulated manufacturers that want near miss reporting inside a full EHS suite with occupational health and AI help on root causes.

  4. 4. EHS Insight

    A modular EHS platform from StarTex Software, offered since 2009. Its Incident Management module records injuries, property damage, vehicle events and near misses, and its manufacturing page describes observation and near miss reports from any worker in about 30 seconds, with a photo and location. Workers report from iOS and Android apps, including offline. Reports move through configurable submit, review and escalate workflows, with 5 Whys and fishbone tools and corrective actions tracked to closure. Dashboards include a near-miss-to-incident ratio, and its Copilot AI helps with report quality and trends. Other modules cover audits, training, compliance and chemicals. Pricing is quoted by modules and headcount, and the small and medium business package starts at $5k per year. It states ISO 27001 certification and hosts on AWS.

    Best for: Small and mid-sized plants that want offline mobile near miss reporting and OSHA records in a modular system with a published starting price.

  5. 5. Intelex

    An EHSQ suite from Intelex Technologies in Toronto, working on EHS software since 1992, with a dedicated Near Miss Reporting application. Workers report from a mobile app online or offline with location, photos and other evidence, on ready-made or custom forms. Near misses above a set severity can start an investigation or CAPA automatically, with email notifications, and investigations use contributing factors, 5 Whys and fishbone. Dashboards show near misses by site, business unit, region and task. Its AI helps write incident descriptions, analyzes photos and looks for patterns. The suite also covers quality, environment, training and audits. Safety Essentials, which includes near miss reporting, starts at $44 per user per month with a 25-user minimum, billed annually; Advanced and Enterprise are quoted. Intelex lists ISO/IEC 27001:2022 certification and SOC 2 and SOC 3 reports.

    Best for: Manufacturers that want a purpose-built near miss app with severity-based escalation and a published entry price, with room to add quality and environmental modules.

  6. 6. Mitti (by SafetyCulture)

    The new name of SafetyCulture, the maker of iAuditor, announced in August 2026. Its Issues feature lets workers report hazards, incidents and near misses from the iOS or Android app, or by scanning an issue QR code without a Mitti account, and AI Issue Creation turns a photo and description into an issue. Critical issues alert their owner, issues become actions with due dates, and Investigations bring related issues and inspections together for root cause and corrective action work. Analytics show issue trends by site, and inspections, training and maintenance share the platform. It publishes pricing: free for up to 10 users, and Premium at $24 per seat per month billed annually or $29 monthly, with Enterprise by quote. Mitti says it is ISO 27001:2022 certified and has a SOC 2 Type II report.

    Best for: Sites that want the lowest-friction reporting, including QR codes for contractors and visitors with no login, at a published per-seat price.

  7. 7. VelocityEHS

    The Accelerate Platform, a suite of more than 24 EHS products; only the near miss parts are described here. Its Incident Management product captures incidents, near misses and hazards in an iOS and Android app that works offline. QR Codes for Incident Management, launched in June 2026, let employees, contractors and temporary workers report with no login, and each submission starts notifications and corrective actions automatically. Its AI analyzes near miss data to flag high-risk hazards, recommends probable root causes and suggests corrective actions. The Observations product handles behavior-based and near miss observations with escalating reminders, and reporting covers trends across locations, with OSHA forms exported electronically. Pricing is not published. VelocityEHS says it is ISO 27001 certified and SOC 2 Type II attested, and supports single sign-on.

    Best for: Manufacturers that want no-login QR reporting and AI that suggests root causes and corrective actions, inside a large EHS suite.

  8. 8. Voxel

    A site intelligence platform that runs computer vision on a facility's existing cameras. Workers don't file reports: the system detects risk events such as forklifts too close to pedestrians or other trucks, speeding, missed stops, PPE gaps and spills. Its vehicles page says this helps prevent near misses from turning into hits, and an article on its site describes a near miss feature that alerts the site team within 60 seconds and logs the time and location. Each event can become an assigned action with follow-up and coaching, and dashboards show trends across sites and a daily summary. Voxel says it works with over 95% of existing IP cameras and deploys in 48 hours. Mobile apps are not described, and pricing is not published. Voxel describes itself as SOC 2 Type II certified with end-to-end encryption.

    Best for: Plants with heavy forklift traffic and existing cameras that want vehicle and pedestrian near misses caught automatically, including the ones nobody reports.
