Training sign-off sheet

A training sign-off sheet records who attended a training session, what it covered, and that each person understood it. This template adds a competence check: after the session the trainer or supervisor confirms each person can do the task to standard, marks them as needing more training, or notes that the session was awareness only. Page 1 has the course or procedure with its document number and revision, the trainer, date, method and what was covered, then 16 attendees with a signature and a competence check. Page 2 has 16 more attendees, the follow-up for anyone who needs more training, and a check a few weeks later that the training changed how the work is done. The Excel version is a training log with one row per person per training, a retrain-by date from how long the training stays valid, and a status for each record.

When to use it

When to use a training sign-off sheet

  • For any training session where you need a record of who attended: classroom, on the job, demonstrations or toolbox talks.
  • When a procedure or work instruction changes and the people who use it must be trained on the new revision.
  • For read-and-sign training on a document, with a check that people can apply it.
  • When an audit or customer asks for training records.

How to fill it in

    Fill in the header

    The course or procedure, its document number and revision, the trainer, date, duration, location and method.

  2. 2

    Write what was covered

    The main points or the procedure steps, and attach the material used.

  3. 3

    Have each person sign

    Each attendee prints their name and signs that they attended and understood.

  4. 4

    Check competence

    The trainer or supervisor watches each person do it, or questions them, and circles pass, more training or N/A.

  5. 5

    Follow up

    Anyone marked for more training gets a next step, an owner and a date. Schedule the people who were absent.

  6. 6

    Check effectiveness

    A few weeks later, check the training changed how the work is done, and update the records and skills matrix.

A filled-in example

Illustrative, not a benchmark

An example: training on revision C of a changeover procedure (illustrative).

  • Course: changeover procedure CO-07, revision C. Trainer: the line lead. Method: demonstration at the machine, 45 minutes.
  • Covered: the new clamp sequence, the torque check with the new wrench, and the first-off check before release.
  • Eleven operators signed. Nine passed the competence check at the machine; two were marked for more practice on the torque check.
  • Both did a supervised changeover the next week and passed.

Three weeks later, changeover audits showed the new clamp sequence followed on every shift, and the skills matrix was updated for all eleven.

Common mistakes

  • A signature as proof of competence

    A signature shows someone attended. Check they can actually do it.

  • No revision on the record

    Without the revision you cannot show who was trained on the current version.

  • Absent people forgotten

    List who missed the session and schedule them.

  • Never checking effectiveness

    Look again a few weeks later: audits, defects or observations show whether the training stuck.
