Preventive maintenance checklist template

A preventive maintenance (PM) checklist lists the planned maintenance tasks for one machine at one interval: what to do, how, to what standard, and what reading was taken. Planned at set intervals, PM catches wear before it causes a breakdown. Page 1 of this template covers the machine, the lockout before you start and 21 task rows with frequency, method or tool, standard, a done box, the reading and initials. Page 2 records parts used, issues found with a work request number and priority, the time the machine was down and sign-off. Page 3 is a yearly PM schedule. The Excel version has a PM schedule that works out the next due date from the last done date and the frequency, and flags PMs that are overdue or due within 7 days, plus a checklist sheet per machine.

When to use it

When to use a preventive maintenance checklist

  • For every machine with planned maintenance tasks at a set interval, daily to yearly.
  • When setting up PM for a new machine from the maker's manual.
  • Where PM is done on paper, or as a backup when your maintenance system is down.
  • To plan the year's PM so the line knows when machines will be down.

How to fill it in

  1. 1

    Write the tasks once

    From the maker's manual and your own experience: task, frequency, method or tool, and the standard or limit for each.

  2. 2

    Lock out first

    Note the lockout procedure, any permit, and that production has been told.

  3. 3

    Do each task and record a reading

    Tick done and write the reading: pressure, temperature, belt tension, clearance. Readings show wear before it fails.

  4. 4

    Log parts and issues

    Parts used, and anything found that was not fixed during the PM gets a work request and a priority.

  5. 5

    Hand back and sign

    Guards on, tools out, locks removed by the people who applied them, the machine tested and accepted by production.

  6. 6

    Update the schedule

    Enter the date done. The next due date is the last done date plus the interval.

A filled-in example

Illustrative, not a benchmark

An example: the monthly PM on an air compressor (illustrative).

  • Task: check the belt tension. Standard: 10 to 15 mm deflection. Reading: 18 mm. Belt tightened to 12 mm.
  • Task: drain the receiver tank condensate. Done.
  • Task: check the discharge temperature at full load. Standard: below 100°C. Reading: 94°C.
  • Issue found: an air leak at a hose fitting on the dryer, not fixed during the PM. Work request raised, medium priority.

The PM took 50 minutes with the compressor down, the schedule moved the next monthly PM to the same date next month, and the leak was fixed that week.

Common mistakes

  • Ticks without readings

    A tick says the task was done. A reading shows the trend that predicts a failure.

  • Tasks too vague

    "Check motor" means different things to different people. Say what to check and the limit.

  • Skipping a PM when busy

    Move a PM rather than skip it. A skipped PM is a late PM until it is done.

  • Issues noted and forgotten

    Anything not fixed during the PM needs a work request, or it waits for the breakdown.
